The “insanity is doing the same thing over and over” etc. quote seems to have been current in alcohol/substance addiction programs in 1980 at least, and presumably in the 1970s; I’ve added another book (apparently originally printed 1980, but Google books has 1992 revised edition so I can’t be sure) that refers to oral use. I’m not sure we can definitively date it (presuming it was in oral use), so the attribution specifically to Narcotics Anonymous (as originator) may be over-stating matters, but these seem (among?) the earliest written versions. It may have roots back to the work of Kelly; I've added a link with a quote there. Discussion elsewhere suggests it may have earlier roots in French, with "Lorsque l'on effectue une action, atteint un mauvais résultat, puis répète l'action plusieurs fois attendent un meilleur résultat, on est un peu fou de singe, non?" having been attributed to Voltaire on a message board, purportedly from a work titled "Reflexions sur de jonque"... which work I'm unable to reliably turn up to find a cite for this bon mot.
